Proof of Vaccination in Public Places in Chicago

Going out to dinner in downtown Chicago or headed to a nearby gym for a workout? Starting January 3, 2022, in addition to your mask, be sure to have your vaccination card handy! Under Public Health Order 2021-2, any individual age five and older must show their proof of vaccination at certain indoor establishments including restaurants, bars, coffee shops, gyms, recreational facilities, entertainment venues, and movie theaters. While you can carry your physical Vaccination Record Card or an official immunization record as proof, this new order also allows you to present a digital photo of your card or record instead. In order to make going out as easy (and safe) as possible, take a photo of your vaccination card and save it as a favorite in your phone so it’s always readily available. Stay safe!
